Creating & publishing a form

Build and publish intake forms.

Quick answer: Go to Forms, create a new form, add your questions, then publish. Published forms can be attached to services or sent to customers after booking.

Overview

The form builder lets you add questions, reorder fields, and preview how customers will see the form. Save as a draft while editing; publish when you're ready to use it.

Start from Forms → Create form.

Tip

Draft a waiver or questionnaire offline first, then copy the questions into the builder.

Step-by-step

  1. Create a new form

    Give the form a clear internal name, e.g. New client health questionnaire.

    Look for: Create form

  2. Add questions

    Add text fields, multiple choice, yes/no, and consent checkboxes. Drag to reorder.

    Look for: Add question

    Mix question types to match your intake process.
  3. Publish the form

    Click Publish when the form is complete. You can unpublish later to stop new submissions.

    Look for: Publish

    Only published forms appear in service attachment dropdowns.
  4. Attach to a service

    Edit a service, open Pricing, and select your published form.

    Look for: Customer form

What happens next

Customers complete the form during booking or from an email link after they book. View responses from the booking detail or your forms dashboard.

Yes. Changes apply to new submissions. Existing responses keep the answers customers submitted.
