Use customer forms to gather information before or after appointments — health questionnaires, liability waivers, preferences, or any custom fields you need.

  • Build forms with text, choice, and consent fields
  • Publish when ready — drafts cannot be attached to services
  • Attach one form per service on the Pricing step
  • Customers complete during booking or via email link
All forms are managed from Forms in the business sidebar.

Before you start

Note

Forms are optional. Many businesses attach a waiver to specific services while leaving others without a form.

How forms fit your workflow

  1. Create a form

    Add questions and save as a draft while you work on it.

    Look for: Forms

  2. Publish the form

    Only published forms can be attached to services or sent to customers.

    Build questions in the form editor, then publish.
  3. Attach to services or send after booking

    Link the form on a service Pricing step, or customers receive a completion link by email.

    Attach forms per service so only relevant customers see them.
